Add multi-tenant RLS pattern to access-control.md (jwt_organization() helper, org-scoped + personal row policies, NULL guard explanation). Extend organization.md with a Using the org claim for RLS subsection linking to the full pattern. Expand pg_session_jwt auth.jwt() entry with o claim extraction examples. Co-authored-by: Isaac

A recent PR shipped the o claim, an org context object added to JWTs when the session has an active organization. Changes:
